Mosaic Community Care has launched a new, interactive online version of its Learn2Adult decision-making roadmap.
Developed with support from Telethon, the digital tool makes Mosaic’s proprietary Learn2Adult resource accessible, readily available, and easy-to-use for teenagers, young adults, and adults with disability, their families, carers and support workers.
The tool covers fifteen areas of life, ranging from education and housing through to hobbies and relationships. It steps young adults through these areas of life to choose where they would like to have more control and make independent decisions. For additional support, Mosaic offers a one-to-one coaching program to help a person navigate the online tool.
The Learn2Adult decision-making roadmap tool is freely available through Mosaic’s website. It’s a starting point for teenagers, young adults, and adults with disability to take control of their lives and become confident decision-makers.
Ongoing support through one-to-one coaching
For those who would benefit from ongoing support, Mosaic offers one-to-one tailored and confidential Learn2Adult coaching sessions.
Tailored coaching sessions are designed to help teenagers, young adults, and adults with using the Learn2Adult decision-making roadmap tool to provide a smoother, less stressful and more effective transition to developing an adult-adult relationship within the family.
One-to-one coaching is available online or face-to-face in your own home, at Mosaic’s offices in Jandakot, or somewhere else that suits the person.
